New Patient Information
Your First Appointment
If you are interested in scheduling a new patient appointment with one of our providers please call the office and fill out all of the forms below:
Fall Risk Assessment Form **only for patients 65 yrs of age and older
Release of Information Form *if you would like information released to your workplace, a family member, another provider, etc.
Complete and return all forms as well as your insurance card (front and back) and a picture ID (via fax or mail). Forms can be uploaded via the link below, or returned to the office in person, faxed, or sent by mail.

Information for New Patient Appointments:
Please arrive 20 minutes prior to your appointment time to fill out paperwork and get registered.
The initial visit will be a comprehensive psychiatric assessment that will take approximately 30 – 45 minutes.
Remember to bring your valid driver’s license or valid state I.D. along with your current insurance card. Please contact your insurance provider prior to your visit to ensure that your clinician is in-network.
Bringing a family member, friend, or caregiver may provide additional helpful information to the clinician.
